To order Reliance Orthodontic Products, simply contact our home office at 1-800-323-4348. There is a minimum order of $10.00

POSTAGE

All prices are F.O.B. Itasca

EXCHANGES OR REFUNDS

Your satisfaction is guaranteed! Reliance Orthodontic Products, Inc. guarantees that its products are of the highest quality and workmanship. If at any time prior to expiration date your adhesive system fails to perform in a manner not consistent with the product, we will replace remaining product with new material free of charge or issue credit for remaining product.

Products which are being returned to Reliance Orthodontic Products, Inc. must have prior return authorization from a Reliance representative.

All Products being returned for credit or replacement must be sent postage paid, with a letter of explanation accompanying merchandise.

TERMS

All orders are accepted on the basis of payment within 30 days after shipment. An invoice is enclosed with each shipment. Payment should be made from this invoice. Any outstanding balance not paid within 30 days will be subject to a finance charge of 11/2% per month, an amount equal to 18% per year. All payments must be made in U.S. funds.

EXPORT

Export orders must be paid for in advance by pro forma invoice.

TERMS OF SALE

Net 30 days. Illinois residents: please add 1.25% sales tax. All invoices payable in U.S. funds. All foreign orders are responsible for shipping costs and must be paid in advance.

PRICES

Prices are subject to change without prior notice. Product improvement and product modification may be introduced without prior notice and without incurring obligation. Shipping charges are paid by the customer.

CONDITIONS

No refunds, credit, or replacement will be issued after 90 days unless product is deemed defective by Reliance.

2. Products which are being returned to Reliance Orthodontic Products, Inc. must have prior return authorization from a Reliance representative and a return authorization number. All Products being returned for credit or replacement must be sent postage paid, with a letter of explanation and return authorization number accompanying merchandise.
